These Terms of Service govern your use of DentalPlanRx ("DentalPlanRx," "we," "us," "our"), a service operated by Amazica Holdings LLC, a Delaware limited liability company. By using this website or joining our waitlist, you agree to these terms.
DentalPlanRx is a membership service that, once launched, will provide virtual dental consultations, AI-assisted oral health screening reviewed by licensed dentists, treatment plan second opinions, access to a partner clinic directory, and discounts on oral care products.
DentalPlanRx is not:
Joining the waitlist signals interest in being notified when DentalPlanRx launches in your state. It is not a membership, does not entitle you to services, and does not create a payment obligation. You may unsubscribe at any time.
You must be at least 18 years old to sign up for DentalPlanRx membership (future). Users under 18 may be covered as dependents under a parent or guardian's membership. You must reside in a U.S. state where our teledentistry partner is licensed to operate.
DentalPlanRx membership is offered on monthly and annual subscription terms. Current pricing is displayed on the website and may change with at least 30 days' notice.
Consultations and second opinions unlock 30 days after your signup date. Partner clinic directory access and product discounts unlock immediately. This waiting period exists to prevent adverse selection abuse and keep membership affordable for all members.
Per our website's detailed pricing page. Current benefits include four virtual consultations per member per year (one per quarter), one Second Opinion per household per year, partner clinic directory access, and member product pricing.
Monthly members can cancel at any time; cancellation takes effect at the end of the current billing period. Annual members can cancel; we do not refund unused months, but access continues through the end of the paid year.
Clinical assessments, diagnoses, and treatment recommendations are provided by licensed dentists employed or contracted by our teledentistry partner. DentalPlanRx is a technology platform and membership organization; DentalPlanRx does not practice dentistry or provide medical advice.
Any consultation, screening, or second opinion provided through DentalPlanRx is based on the information you submit. The results may not be fully accurate if information is incomplete or inaccurate. Hands-on in-person examination may be required for certain conditions.
Use of DentalPlanRx services does not create a dentist-patient relationship with DentalPlanRx. It creates a patient relationship with the teledentistry partner's clinical entity.
DentalPlanRx's partner clinic directory lists third-party dental practices that have signed participation agreements with DentalPlanRx. DentalPlanRx does not operate these clinics, employ their staff, or guarantee their work. We monitor partner compliance with our participation agreement through two-way ratings and remove partners who violate the agreement after a three-strikes process.
Shop products are manufactured and distributed by third-party brands (GuruNanda, Hello, Parodontax, Waterpik, Oral-B, and others). Product statements have not been evaluated by the FDA. Products are not intended to diagnose, treat, cure, or prevent any disease. Brand names and product images are used for product identification purposes only. DentalPlanRx makes no claim of authorized-retailer status with any brand unless explicitly stated.
Shop launches after DentalPlanRx opens for paid memberships. Pricing displayed during the waitlist phase represents planned pricing at launch and is subject to change.

The DentalPlanRx name, logo, site design, content, and software are owned by Amazica Holdings LLC. You may not use them without our written permission. Third-party brand names and images displayed on DentalPlanRx are the property of their respective owners.
DentalPlanRx is provided "as is" and "as available." We do not guarantee uninterrupted service, error-free operation, or specific clinical outcomes. To the maximum extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement.
To the maximum extent permitted by law, Amazica Holdings LLC, its officers, employees, and agents are not liable for indirect, incidental, consequential, or punitive damages arising from your use of DentalPlanRx. Our total liability for any claim is limited to the amount you paid DentalPlanRx in the 12 months preceding the claim.
These terms are governed by the laws of the State of Delaware, without regard to conflict of laws principles. Disputes will be resolved through binding arbitration in Delaware, except for small claims matters or injunctive relief.
We may update these terms to reflect changes in our service or legal requirements. Material changes will be communicated via email to registered users at least 30 days before taking effect. Continued use after the effective date constitutes acceptance.
