Privacy Policy

DentalPlanRx ("DentalPlanRx," "we," "us," "our") is a service operated by Amazica Holdings LLC. This privacy policy explains how we collect, use, and protect your information.

1. Information we collect

When you join the waitlist: your email address, optional first name, and optional indication of interest area. We also log IP address for security and fraud prevention.

When you become a paying member (future): billing information (handled by Stripe, not stored by DentalPlanRx), account credentials, membership status, and household composition you provide.

When you use clinical services (future): any information you submit to the teledentistry partner for your consultation, including symptoms, photos, and related health information. This information is considered Protected Health Information (PHI) under HIPAA.

Automatic collection: standard server logs (IP address, browser type, pages visited, timestamps) for security, debugging, and aggregate analytics.

2. How we use your information

• To notify you when DentalPlanRx launches in your state (waitlist).
• To provide the service you signed up for (membership, consultations, shop).
• To improve our service, diagnose technical issues, and prevent abuse.
• To comply with legal obligations.

We do not sell your data. We do not share it with advertisers. We do not use it to train advertising profiles.

3. Third parties

We share information only with vendors necessary to provide our service, under strict data processing terms. Current vendors include:

• Hosting: Bluehost (web hosting and database).
• Payments (future): Stripe (payment processing; we never store card numbers).
• Email (future): Brevo (transactional and marketing email).
• Clinical services (future): Our teledentistry partner (name disclosed once contracted), which holds Protected Health Information under a Business Associate Agreement.

Each vendor is contractually restricted to using your data only to provide their service to us.

4. Your rights

You can:

• Request a copy of the data we hold about you.
• Request correction of inaccurate data.
• Request deletion of your account and associated data (subject to legal retention requirements for billing records and PHI).
• Unsubscribe from marketing emails via the link in any email.

5. Cookies

We use only essential cookies for session management and form security. We do not use advertising cookies or third-party tracking pixels on the marketing site.

6. Children

DentalPlanRx is not intended for use by children under 13 without a parent or guardian. We do not knowingly collect information from children under 13. If you believe we have, contact us to delete it.

7. Security

We use industry-standard security measures including HTTPS, encrypted database storage, access controls, and secure password storage. No system is 100% secure; we commit to notifying affected users if a breach involving their personal information occurs.

8. HIPAA Notice of Privacy Practices

This section applies once you become a paying DentalPlanRx member and use clinical services. Until then, DentalPlanRx's marketing site collects only non-PHI information (email, name, IP).

DentalPlanRx and our teledentistry partner are a Covered Entity / Business Associate relationship under the Health Insurance Portability and Accountability Act. We handle your Protected Health Information (PHI) as follows:

Uses of your PHI

• Treatment: Your dentist uses your PHI to evaluate and provide clinical services.
• Payment: We use minimum necessary information to bill for services.
• Operations: Quality improvement and de-identified analytics.

We do not use PHI for marketing without your explicit authorization. We do not sell PHI.

Your rights under HIPAA

• Right to access your medical records.
• Right to request corrections.
• Right to request restrictions on how your PHI is used.
• Right to request confidential communications.
• Right to an accounting of disclosures.
• Right to file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights without retaliation.

Data security

PHI is stored on HIPAA-compliant infrastructure with encryption at rest and in transit, access controls, and audit logs. Our teledentistry partner maintains its own HIPAA-compliant infrastructure governed by a Business Associate Agreement between DentalPlanRx and the partner.

9. Changes to this policy

We may update this policy to reflect changes in our service or legal requirements. Material changes will be communicated via email to registered users. The "Last updated" date at the top of this policy reflects the most recent revision.
